A Personal Care Aide can come to the home and help with bathing, dressing, cooking, cleaning, running errands, and more.

Coordination of care

A Care Management Team, led by a Registered Nurse or a Licensed Social Worker, will help coordinate a care plan so the patient, doctors, and family/caregivers are all on the same page.

This highly rated all-in-one plan combines the medical coverage of a Medicare Advantage plan with the in-home benefits of Medicaid Long-Term Care (LTC). CompleteCare offers a $252/month OTC Plus card, which can be spent on health-related items, exercise equipment, activity trackers, healthy foods, and even home utilities (e.g., gas, oil, electric, water, and internet service).*

If you receive Medicaid and want to know whether you are eligible for a Managed Long-Term Care Plan, you can call the New York Independent Assessor Program (NYIAP) at 1-855-222-8350, Monday to Friday, 8:30am–8pm, or Saturday, 10am–6pm, to schedule a free evaluation.

*These are special supplemental benefits for members enrolled in a Healthfirst dual-eligible special needs plan. To be eligible, you must have a chronic condition such as cardiovascular disorder, disabling mental health condition, diabetes, lung disorders, obesity, or another eligible condition not listed. Benefit eligibility is not based on your chronic condition alone. Benefits may vary by plan.

Coverage for Senior Health Partners Managed Long-Term Care Medicaid Plan is provided by Healthfirst PHSP, Inc. Plans above reflect amounts after Extra Help or Medicaid secondary coverage has been applied.

Coverage is provided by Healthfirst Health Plan, Inc., which offers HMO plans with a Medicare contract. Our SNPs also have contracts with the NY State Medicaid program. Enrollment in Healthfirst Medicare Plan depends on contract renewal. Plans contain exclusions and limitations.
