Get a ride to the doctor

If your plan includes transportation to/from your medical appointments, your car ride can be on us.

Simply call Modivcare at 1-844-772-6629, Monday to Friday, 8am–8pm, or use the Modivcare app to schedule, cancel, or check the status of a ride.

Be sure to schedule your ride two days in advance and plan to arrive 30 minutes before your doctor’s appointment time.

To see how many one-way trips your plan offers each year, and how many trips you have left, log in to the Member Portal or use your Healthfirst NY Mobile App.

Order medical supplies and equipment

Medical devices require a prescription from your doctor, and in some cases a prior authorization, which your doctor can submit on your behalf.

Your doctor will know if a prior authorization is needed and can take care of it for you.

COVID-19 vaccinations and testing

Healthfirst covers COVID-19 testing and vaccination when provided by in-network providers.

COVID-19 at home tests are not a covered benefit for most Healthfirst plans; however, Healthfirst members may use their OTC Plus card to purchase at-home tests from participating retailers.

Disenrolling from your Healthfirst Medicare Advantage plan

While we hope your experience with Healthfirst has been a positive one, we understand that circumstances may require you to disenroll from your plan, whether voluntarily or involuntarily.

If you are disenrolling from your Healthfirst Medicare Advantage plan, your medical and prescription drugs benefits will continue through the effective date of your disenrollment. During this time, you will remain responsible for any applicable cost-sharing, including copayments, coinsurance, and premiums related to your Healthfirst benefits.

For more detailed information about disenrollment, including your rights and responsibilities, please refer to the “Ending Membership in Our Plan” section of your plan’s Evidence of Coverage (EOC).

Coverage is provided by Healthfirst Health Plan, Inc., Healthfirst PHSP, Inc., and/or Healthfirst Insurance Company, Inc. (together, “Healthfirst”). Healthfirst Medicare Plan has HMO and PPO plans with a Medicare contract. Our SNPs also have contracts with the NY State Medicaid program. Enrollment in Healthfirst Medicare Plan depends on contract renewal. Plans contain exclusions and limitations.

OTC items are subject to the plan’s list of eligible items and the plan’s participating network of retail, online, and utility providers. Modivcare and Medical Answering Services provide the covered transportation services under your plan.
