The Senior Health Partners Managed Long-Term Care Medicaid Plan provides you or your loved one with coordinated care that includes long-term benefits, such as personal care services and adult day healthcare. You’ll also get a Care Team, led by a Registered Nurse or a Licensed Social Worker, that will (1) help you develop your own personal care plan centered around your care goals and (2) coordinate the services you need to meet your care goals and remain independent.

If you have medical and prescription drug coverage from a Medicare plan and need added long-term care services, you can get all these benefits as well as additional benefits like an OTC Plus allowance to use for over-the-counter medications, healthy foods, home utilities, and more*—all in one integrated plan. Costs and plan details

Covered services for the plan include:
  • Care management team
  • Personal care services
  • Adult day healthcare
  • Skilled nursing home care
  • Home-delivered meals
  • Personal emergency response system (PERS)
  • Hearing, dental, and vision services
  • And more!
You may be eligible for Senior Health Partners if you:
  • Are 18 years of age or older
  • Reside in the plan’s service area, which is New York (Manhattan), Bronx, Kings (Brooklyn), Queens, Richmond (Staten Island), Nassau, or Westchester counties.
  • Are Medicaid eligible
  • Have the ability, at the time of enrollment, to return to or remain in your home and community without danger to your health and/or safety
  • Are in need of Community Based Long-Term Services and Supports (CBLTSS) for more than 120 days from the effective date of enrollment and meet the minimum needs requirements as follows:
    • At least limited assistance with physical maneuvering with more than two activities of daily living (ADL); or
    • Individuals with a dementia or Alzheimer’s diagnosis assessed as needing supervision with more than one ADL
For those continuously enrolled in a Managed Long-Term Care Plan (MLTC), Medicaid Advantage Plus Plan (MAP), and PACE as of August 2025 do not need to meet the minimum needs criteria.

If this is your first time receiving Community Based Long-Term Services and Supports (CBLTSS) you must contact the New York Independent Assessor Program (NYIAP) to find out if you are eligible. *Healthy foods and home utilities are special supplemental benefits for members enrolled in a Healthfirst dual-eligible special needs plan. To be eligible, you must have a chronic condition such as cardiovascular disorder, disabling mental health condition, diabetes, lung disorders, obesity, or another eligible condition not listed. Benefit eligibility is not based on your chronic condition alone.

Coverage for Senior Health Partners Managed Long-Term Care Medicaid Plan is provided by Healthfirst PHSP, Inc.

Coverage is provided by Healthfirst Health Plan, Inc., which offers HMO plans that contract with the Federal Government. Enrollment in Healthfirst Medicare Plan depends on contract renewal.
