Healthfirst Long-Term Care Plans offer health benefits that help you (or your loved one) remain at home and as independent as possible. They can provide help with day-to-day needs and coordinate care with you/your loved one, doctor, and family. But the process of getting a plan can be complicated. We can help make it easier with these three steps.

This all-in-one plan combines Medicare Advantage and Medicaid Long Term Care services AND medical health coverage. So members are covered for in-home care PLUS doctor visits, prescription drug coverage, and a $280 a month over-the counter (OTC) Plus card allowance to spend on health-related items, healthy foods, fitness equipment and trackers, and even home utilities (e.g. gas, oil, electric, heat, and internet service).*
This plan adds access to in-home Long-Term Care services to existing health coverage. You would use your Medicare and/or Medicaid health coverage to access medical, hospital, and prescription drug coverage.

**Non-Emergency Medical Transportation is provided by the New York State Transportation Broker: Medical Answering Services (MAS). MAS can be reached at: 1-844-666-7740 for those living in New York City, Nassau, and Westchester Counties, for those in all other counties dial 1-866-932-7740.
If you receive Medicaid and want to know whether you are eligible for a Managed Long-Term Care Plan, you can call the New York Independent Assessor Program (NYIAP) at 1-855-222-8350, Monday to Friday, 8:30am–8pm, or Saturday, 10am–6pm, to schedule a free evaluation.
Coverage for Senior Health Partners, Managed Long-Term Care Plan, is provided by Healthfirst PHSP, Inc. Plans above reflect amounts after Extra Help or Medicaid secondary coverage has been applied.
Coverage is provided by Healthfirst Health Plan, Inc., which offers HMO plans with a Medicare contract and a contract with the NY State Medicaid program. Enrollment in Healthfirst Medicare Plan depends on contract renewal. Plans contain exclusions and limitations.
*For Healthfirst CompleteCare: Healthy foods/groceries and home utilities are special supplemental benefits. To be eligible for these benefits, you must have one of the following chronic conditions: cardiovascular disorder, disabling mental health condition, diabetes, lung disorders, or obesity. There are other eligible conditions not listed. Your chronic condition alone cannot guarantee eligibility for these benefits
