COVID-19 testing and vaccination is covered with the same cost-sharing as other vaccinations and diagnostic tests, subject to your plan’s deductible, copay, and/or coinsurance, as described in your Summary of Benefits and Coverage and subscriber contract. Note: COVID-19 testing and vaccination provided by out-of-network providers is not covered. You can get your vaccine cost-sharing information online at MyHFNY.org or by calling the Member Services phone number on your Member ID card.
