Choose your primary care provider

If you haven’t chosen your primary care provider (PCP) yet, please do so right away. If you’re not sure, you can call Member Services and we can see if you already have one and help you choose one if you don’t.

When choosing a PCP:

  • Choose a doctor, nurse practitioner, or other healthcare provider within the Healthfirst network. You can find a full list of providers at HFDocFinder.org.
  • Call the doctor’s office to make sure they are taking new patients.
  • You can choose a different PCP for each family member or use one for the whole family.

Switch your primary care provider

Step 1:

Visit HFDocFinder.org to find a doctor in the Healthfirst network

Step 2:

Change your PCP via:

  • The Healthfirst NY Mobile App

    Search for a new PCP under Primary Care. Then tap “Change My Primary Care Provider”
  • The Member Portal at MyHFNY.org
    Log in to your account, go to the “Physicians and Facilities” page, and click “Change PCP Here”

If you prefer to speak with someone, call the Member Services number on your Healthfirst ID card or visit a local Healthfirst Community Office in your area.

Use your dental benefit

Make an appointment for your semi-annual dental checkup and cleanings. Go to HFDocFinder.org or log in to MyHFNY.org to find an in-network dentist. You can change your dentist at any time.*

Choose the best option for care

Your plan gives you many options for care. Selecting the right in-network care option can save you time and money:

Retail health clinics

Generally within a retail store and staffed by nurse practitioners or physician assistants. For quick, less complex health needs.

Urgent care

Immediate walk-in care for non-emergency situations: available nights, weekends, and holidays.

Emergency room

Immediate care for serious or life-threatening conditions; available 24/7.

As always, find an in-network doctor by visiting HFDocFinder.org or by logging in to MyHFNY.org.

Order medical supplies and equipment

Medical devices require a prescription from your doctor, and in some cases a prior authorization, which your doctor or medical equipment provider can submit on your behalf.

Your doctor will know if a prior authorization is needed and can take care of it for you.

Your plan may include supplies such as:

Continuous Glucose Monitor (CGM)

Healthfirst makes it easy to get your CGM. Member Services will help connect you with an in-network CGM provider. Once a provider is identified, Healthfirst will send the prior authorization directly to them. The provider will then send your CGM to you.

Diabetic testing supplies

With a prescription, you can get a Contour Next meter and test strips at an in-network pharmacy.

Medicaid Managed Care and Personal Wellness Plan members receive medical supplies and equipment through Medicaid NYRx.

View personalized benefits

By logging in to your Member Portal or Healthfirst NY Mobile App, you can see your specific plan benefits, as well as details about what benefits you’ve used and what benefits are still available for the year.

You’ll be able to:

  • View/change your PCP
  • Find a doctor, pharmacy, or urgent care
  • View/share your digital Member ID card
  • Access Teladoc Health® and DentaQuest
  • View Community Resources
  • Enroll in paperless communications
  • Get updates about your plan

COVID-19 vaccinations and testing

Healthfirst covers COVID-19 testing and vaccination when provided by in-network providers.

COVID-19 at home tests are not a covered benefit for most Healthfirst plans.

*Leaf Plan members have pediatric dental and vision benefits for children up to age 19. Leaf Premier Plan members have dental and vision benefits for all ages.

Coverage is provided by Healthfirst PHSP, Inc. Plans contain exclusions and limitations.

Telemedicine (Teladoc Health) isn’t a replacement for your primary care provider (PCP). Your PCP should always be your first choice for care (both in-person and virtual visits).
