How to renew your Healthfirst health insurance coverage

Many health plans require you to renew your plan each year in order to keep your health coverage and we’re here to help you every step of the way. We can help make sure you don’t go even one day without the coverage you need for doctor visits, prescriptions, and more. Here’s what you need to do.

And remember, your plan eligibility can change each year based on your income, dependents, and more.

Are you receiving Supplemental Security Income (SSI) benefits?

Social Security Income (SSI) provides monthly payments to people with disabilities, and older adults who have little or no income or resources. Adults and children might be eligible if they have a disability, blindness, or are age 65 or older. Please note, Social Security Income (SSI) is different from Social Security Disability Insurance (SSDI), with eligibility being based on disability and work credits. If you receive Social Security Disability Insurance (SSDI) and have Medicaid coverage, you may need to renew your coverage each year.

Some plans may require extra documents for renewal

You may need to have:

  • Immigration documents (if any information has changed) such as a U.S. passport, naturalization certificate, a permanent resident card, employment authorization, etc.
  • Proof of household income such as most recent paystubs (four if you’re paid weekly, two if you’re paid biweekly), your current signed income tax return and all schedules if self-employed, a letter from your employer, unemployment award letter, etc.
  • Family information (if any information has changed) such as birth dates, Social Security numbers, birth certificate, passport, or permanent resident card
  • Social Security award and pension award letter (if applicable)

Here’s how to renew your Medicare Advantage with Medicaid plan with Healthfirst

Step 1: Know your renewal date

Not everyone’s date will be the same. You can find yours on the Healthfirst NY Mobile App and MYHFNY.org.

Or, call us, and we'll help you find your renewal date. We can also make an appointment for you to speak with a representative. We're working with SPS Community Solutions to help Healthfirst Medicare Advantage members with their Medicaid renewals.

Keep in mind that your completed application is due before your coverage ends, and that different plans have different deadlines.

Step 2: Watch for communications

You'll receive your paper renewal application in the mail from the Human Resources Administration (HRA) or your Local Department of Social Services (LDSS). Make sure your current mailing address, phone number, and email address are up to date so you can be reached by the appropriate agency. Contact New York City’s Human Resources Administration or your county’s Medicaid office to update your contact information.

Healthfirst will also reach out to you by mail, email, text, or phone to make sure you don’t miss your Medicaid plan renewal date. You can update your contact information with Healthfirst:

  • On the Healthfirst NY Mobile App: Go to Profile, then Account Details, then Contact Information.
  • On MYHFNY.org: Go to Account Settings, then Personal Information, then Contact Information.

Please note: If you received a paper renewal packet in the mail at two different times, New York State and CMS advise you to complete, sign, and return both renewal applications in order to keep your coverage active.

Step 4: Medicare Savings Program

If you're on Medicare, your eligibility can change from year to year based on income, dependents, and more. New rules mean you can now have a higher income and be eligible for the Medicare Savings Program. That can save you a lot of money, so call Healthfirst and see if you qualify.

Some plans may require extra documents for renewal

You may need to have:

  • Proof of Income (Social Security Award Letter, Pension Statement, or pay stubs).
  • Proof of Residency (Rent bill, rent receipt, letter from landlord verifying where you live and your rent amount, mortgage statement or utility bill).
  • Proof of Resources (Checking/Savings Accounts, Life Insurance, Annuity/IRA/Stock and Bonds).
  • Proof of Supplemental Health Insurance (i.e. AARP, BCBS) (A copy of your monthly/quarterly bill and proof of payment).
