Leaf/Leaf Premier and Pro/Pro Plus EPO Members

Beginning Jan. 1, 2024, COVID-19 testing and vaccination will be covered with the same cost-sharing as other vaccinations and diagnostic tests, subject to your plan’s deductible, copay, and/or coinsurance, as described in your Summary of Benefits and Coverage and subscriber contract. Note: COVID-19 testing and vaccination provided by out-of-network providers will not be covered, beginning Jan. 1, 2024. You can get your vaccine cost-sharing information online at or by calling the Member Services phone number on your Member ID card.
