How do I apply for Medicaid?
You can:

  • Call us at 1-866-463-6743, Monday-Friday, 8am-6pm.
  • Visit one of our many Healthfirst Help Centers where a representative will help you understand your health plan needs and advise you on the options that are right for you.
What happens after I send in my Medicaid application?
  • A Healthfirst representative will contact you to let you know if you and/or your family can get Medicaid. If you can, we will send your application to your local Department of Social Services for review.
  • Once confirmed, you will get a welcome letter from Healthfirst with your start date, a member handbook that includes benefits information, and a member ID card.
    • If you need healthcare before your member ID card arrives, you can use the Healthfirst welcome letter to show your doctor that you have health insurance.
What do I need to apply for Medicaid through Healthfirst?
  • Proof of age, such as your birth certificate
  • Proof of United States citizenship or alien status*
  • Recent paycheck stubs (if you are working)
  • Proof of income, from any of the following:
    • Social Security
    • Supplemental Security Income (SSI)
    • Veterans' Benefits (VA)
    • Retirement
  • Proof of where you live. This can include any of the following:
    • Rent receipt
    • Landlord statement
    • Mortgage statement
    • Mail with your address on it
  • Health insurance benefit card or the policy (if you have any other health insurance).
*Medicaid may cover you, even if you have alien status. You must be pregnant, or need emergency treatment. This must be certified by a doctor. Other requirements may apply.
How do I know if I can get Medicaid through Healthfirst?
You may be covered by Medicaid if you:  
  • Meet certain income requirements
  • Live in the Healthfirst service area:
    • Bronx, Brooklyn, Manhattan, Queens, Richmond, Nassau and Suffolk counties
  • Are a legal U.S. citizen

What is Healthfirst Medicaid?

Do I need to renew my Medicaid Managed Care coverage?

Yes. Your Medicaid Managed Care plan generally expires one year after you signed up, and you need to renew your health plan every year.

The easiest and fastest way to renew your Medicaid coverage is to call the Healthfirst representative who helped you enroll, visit one of our Help Centers, or go online to schedule an appointment.

You will receive a notice from either the NY State of Health (NYSOH), the Human Resources Administration (HRA), or your Local Department of Social Services (LDSS) before your anniversary date. Make sure you open and read the notice to get the details about renewing your health insurance plan! Your coverage will be cancelled if you don’t renew when you are required to or don’t return documents by the requested date.
