To be eligible for a Healthfirst Medicare plan, you must have both Medicare Part A and Medicare Part B.

To be eligible for a Healthfirst Medicare plan, you cannot have End-Stage Renal (kidney) Disease (ESRD).*

*Please note there are two exceptions to the ESRD restriction:

1. If the individual no longer receives dialysis and can show written proof of this, he/she may be eligible for a Healthfirst Medicare Advantage plan.

2. If the individual had another Medicare advantage plan and was involuntarily disenrolled, he/she may be eligible for a Healthfirst Medicare Advantage plan.
